Silent aura without headache: what that actually is

ngl when i got the full aura - scotomas, that shimmer starting at the edge of my vision, the creeping darkness - and then nothing, i thought something was wrong with me. no headache. no pain. just the warning signs and then silence.

turns out that’s a real migraine variant. you’re still having the neurological event (that’s what aura is), but the headache part didn’t fire. it’s sometimes called silent migraine or acephalgic migraine. not common, but it happens, especially if you’re on a preventive like Ajovy or in a hormonal window where your prevention isn’t covering everything.

the scary part is the uncertainty, the waiting for pain that doesn’t come. but you’re not dying and nothing’s wrong. aura without headache is in the diagnostic books as a legitimate migraine type, even though it doesn’t fit the “migraine equals pain” equation most people have in their heads. what i do now: i treat it like a real migraine warning even without pain.

i take my abortive during the aura because the neurological event is still happening. some people skip the abortive if there’s no pain, but i’d rather be safe on that one. maybe that’s overly cautious, but it’s what works for me. has this happened to you?

did you treat it like a migraine attack or did you wait to see if pain showed up?
