Sema constipation: my Miralax protocol

Hit the constipation side of semaglutide around month three, same as most people. The advice online was a daily capful of Miralax. I’m not a fan of daily anything if it’s not necessary, so I decided to treat it like a protocol and find the minimum effective dose. My approach was simple.

I started with a quarter cap on day 3 post-injection, which is when things usually slow down for me. I logged water intake separately. This is the part I think people miss. Miralax is an osmotic.

It works by pulling water into the colon. If you don’t increase your total water intake, it’s just borrowing that water from the rest of your system. For someone like me trying to keep a bad knee happy, systemic dehydration is a non-starter. I tracked three things: dose, water intake over baseline, and outcome.

A quarter cap did nothing. A half cap on day 3 and day 5, with an extra liter of water on those days, worked. Things moved without bloating or emergency. A full cap just caused cramping.

The key was decoupling the Miralax from the water. They’re two separate variables. Taking the powder without adding the water just made me feel dry and my knee felt stiff the next morning. I log all of it.

the osmotic piece is spot on and the part most people miss. i’d push back a little on framing them as “two separate variables” though. they’re not really separate, they’re just two inputs into a single codependent mechanism. the miralax isn’t doing anything without the water, it’s just inert powder until it has fluid to pull into the colon.

thinking of it as one system with two required inputs helped me get my head around it. fwiw, the other thing to track is magnesium. a lot of people jump to mag citrate (another osmotic) but mag glycinate at night can sometimes help with the underlying muscle contraction part of the equation, which is a different issue than just transit time. the two get conflated a lot.

your logging is way more disciplined than mine. i just dump my dose, water intake, and a nausea score into the daily check-in on CareClinic and hope a pattern shows up later when i have enough brain cells to look at a chart. which is rarely before 9pm. ymmv of course.

running this like a proper protocol is exactly the right approach. Your point about decoupling the Miralax from the water is the part that usually gets missed. People treat it like a single intervention, but they’re two separate variables.

Taking the osmotic without providing the extra water just pulls it from elsewhere, and your “knee felt stiff” observation is the perfect example of that systemic effect… When I ran sema for a few months, I found magnesium glycinate was a better tool for me personally, but the method was the same: track the dose, track the water, track the outcome. The logging is the only way to see the pattern…
The weekly summary view in the app I use (CareClinic) is what finally made it obvious that my water intake was consistently dropping on day 2 post-injection, which was the actual problem.
