Ocrevus fatigue spike

been away for a bit, back now. so i’ve been on ocrevus for almost 2 years and the fatigue has gotten worse lately.

i’m talking exhaustion after a 30min drive, which is crazy for me bc i’m a delivery driver. anyone else on ocrevus experience this kind of fatigue spike?

did you switch dmts or find something that helped?

Fatigue after short drives is a red flag. “exhaustion after a 30min drive” really stands out to me because I’ve been there with my own autoimmune stuff.

The part that gets me is how it sneaks up - one day you’re fine, the next you’re crashing after something that used to be no big deal. I’ve found that tracking my energy levels and labs over time helps me see patterns I wouldn’t have noticed otherwise.

For me, it’s all about the C3 and C4 numbers - when they drop, I know I’m in for a rough ride. Have you talked to your doc about adjusting your treatment plan or adding anything to help with the fatigue?

Ocrevus fatigue sounds rough, did you try adjusting your methotrexate dose to see if that helped with the exhaustion after driving, you mentioned “exhaustion after a 30min drive” which is pretty specific and i’m wondering if that’s related to your medication or something else entirely, like your rheumatologist suggested a prednisone taper but that doesn’t seem to be helping with the fatigue, and i’m also curious if you’ve noticed any patterns with your joint pain or photosensitivity since starting ocrevus, or if it’s just the fatigue that’s been a major issue for you lately.

did your neuro run new labs after the fatigue spike?

that kind of fatigue spike after two years, especially when ocrevus has a fairly stable pharmacokinetic profile, suggests the change is more likely driven by subtle disease activity or other comorbidities than by fluctuations in drug levels. it’s worth having your neuro look at the whole clinical picture again.
