Mitochondrial transplants in n=2: the gap between feeling better and knowing why

honest take: the mechanism is interesting, but “improved energy” in two elderly patients on right-to-try doesn’t tell us much yet. i’m not being flip - i want to sit with this. they feel better. that matters. but in populations where desperation is high and n is 2, we’re measuring… what exactly? restoration of mitochondrial function? placebo with a compelling story? natural variation? the distinction matters for efficacy trials coming mid-2026. “our patients reported improved energy” is a starting signal, not a mechanism. energy has multiple sources - inflammation, sleep, expectations, hormonal changes, reduced pain. any of those could shift in elderly patients receiving attention and hope. the move here is to stay curious without confusing “promising” with “proven.” we don’t have the data yet. the phase 1 is supposed to tell us if it’s safe and worth studying further, not if it’s effective.
