Is it possible to change the name of a care plan

I want to name a care plan for my father and a separate plan for my mom

Under More, select My Care Plans, then tap the > to the right of the plan that you wish to rename. Tap the gear icon in the upper left corner and choose Edit Care Plan. The Plan Name and Plan Description can then be modified on the Edit Plan screen
