when i was at 15-18 migraine days a month, my neurologist said “episodic” and moved on. now i’m at 8-10 days on topiramate and she still says episodic because i have some migraine-free days. but like… the actual difference between predictable flares and constant threat-mode isn’t just a label thing.
episodic meant i could see the pattern. first tuesday of the month, always. high pressure systems, always. i logged months of data and it showed me trigger windows - like if i skip coffee AND don’t sleep well, i get hit in the next 36 hours.
that data was usable. chronic feels different. it’s not just more days, it’s like my nervous system quit sending a warning. when i was on nothing but triptans, missing 8 attacks in 10 days felt random.
the logging became noise. there’s no window to track. the meds are the same either way. Botox, preventive, abortive - doesn’t matter if you’re technically episodic or chronic.
but the way you experience your own body? completely different. one feels managed, one feels like surrender. does this distinction actually change how you approach treatment, or is it just a doc checkbox?
anyway.
