Billing/Account issue

About a week ago, I believe, I purchased a discounted, 49.99$ yearly membership on the CareClinic tracker app on an iphone. Yesterday, I received a bill from Apple for the regular price of 59.99$, and today the app is once again asking me to purchase a plan. It is additionally disheartening that I couldn’t find any email contact or billing support and had to make another account on this public forum to look for help. Please let me know if you are able to provide that support, or if there is another point of contact. Thank you kindly.
